Mirtazapine is an α2 receptor antagonist acting on the central presynaptic, enhancing adrenergic nerve conduction. It regulates the function of 5-HT by interacting with the central 5-hydroxytryptamine (5-HT2, 5-HT3) receptors. Both optical enantiomers of mirtazapine have antidepressant activity. The left-handed isomer blocks α2 receptors and 5-HT2 receptors, and the right-handed isomer blocks 5-HT3 receptors. Mirtazapine has a sedative effect, good tolerance, and almost no anticholinergic effect. Its therapeutic dose has no effect on the cardiovascular system.
It is quickly absorbed after oral administration, with a bioavailability of about 50%. The plasma concentration reaches a peak after about 2 hours, and the plasma protein binding rate is about 85%. The average half-life is 20 to 40 hours, occasionally up to 65 hours, and a shorter half-life is occasionally seen in young people. The blood drug concentration reaches a steady state 3 to 4 days after taking the drug, and there will be no aggregation in the body thereafter. Its main metabolic mode is demethylation and oxidation reaction, and the metabolites after demethylation are still pharmacologically active like the original compound. Mirtazapine is excreted in urine and feces within a few days of taking the drug. Poor liver and kidney function can cause a decrease in the clearance of mirtazapine.
Used to treat various depressions. It is effective for symptoms such as anhedonia, psychomotor depression, poor sleep (early awakening), and weight loss. It can also be used for other symptoms such as loss of interest in things, suicidal thoughts, and mood swings (good in the morning, bad in the evening). This drug takes effect after 1-2 weeks of medication.
