Bivalirudin is primarily indicated in the following settings:
Percutaneous transluminal coronary angioplasty (PTCA): For patients with unstable angina undergoing PTCA.
Percutaneous coronary intervention (PCI): For patients undergoing PCI, especially in the REPLACE-2 study, in combination with a temporary platelet glycoprotein IIb/IIIa receptor antagonist (GPI).
Heparin-induced thrombocytopenia (HIT) or heparin-induced thrombocytopenia and thrombosis syndrome (HITTS): For patients at risk of the above undergoing PCI.

Bivalirudin exerts its anticoagulant effect by directly inhibiting the activity of thrombin, and its inhibitory effect is reversible and transient. Compared with ordinary heparin, Bivalirudin does not need to rely on antithrombin to exert its anticoagulant effect, so it does not cause problems such as thrombocytopenia.
